My Community Health Center is proud to offer a convenient and secure Patient Portal powered by FollowMyHealth. This easy-to-use online tool allows you to stay connected with your healthcare team and manage important health information anytime, anywhere.
Getting started is simple - all you need is a valid email address on file with our office. Once you are enrolled, you can log in from your computer, tablet, or smartphone to access your personal health information.
Through the Patient Portal you can:
